Improving outcomes across all patient groups requires an organizational commitment and should be a strategic priority. This requires a leader(s) identified by the organization to provide oversight and coordination of activities to reach its goals. Leader(s) must also have a thorough understanding of the organization’s patient population so that the care provided is relevant to the population’s needs.
